Home Sweet Home Care is seeking an experienced and dedicated Care Coordinator & Assessor to join our growing team. This is a field-based role requiring regular travel within your designated region. The successful candidate will play a key role in assessing client needs, supporting care staff, and ensuring the delivery of high-quality, person-centred care services.

Essential Requirements
Full UK driving licence and access to your own vehicle.
Previous experience in domiciliary care.
Previous experience as a Care Coordinator.
Good understanding of the homecare sector.
Excellent communication, organisational, and problem-solving skills.

Desirable Requirements

Previous experience or knowledge of complex care services.
Key Responsibilities
Client Assessments and Care Planning
Promote the services provided by Home Sweet Home Care.
Conduct initial assessments of care needs for prospective clients.
Complete associated risk assessments.
Develop person-centred care plans that accurately reflect clients' needs, goals, preferences, and aspirations.
Review and update care plans as required.
Staff Support and Development
Build and maintain positive relationships with care staff.
Ensure staff supervision, competency assessments, and support visits are completed and remain up to date.
Support the Care Deputy Manager and Care Coordinator in maintaining high standards .
Care Coordination
Manage and monitor staff schedules using rostering software to ensure seamless care delivery.
Reallocate visits and shifts effectively to cover absences and service demands.
Monitor attendance and punctuality using electronic monitoring systems.
Ensure staffing levels are maintained to meet client requirements.
Communication and Partnership Working
Liaise with families, social services, healthcare professionals, and other stakeholders to ensure care provision remains appropriate and effective.
Report significant changes in service users' health, wellbeing, or support needs to the relevant manager.
Maintain accurate records using company systems and processes.
Compliance and Quality Assurance
Adhere to company policies, procedures, and regulatory requirements.
Promote confidentiality, safeguarding, equality, diversity, and inclusion.
Participate in relevant training and professional development activities.
Contribute to maintaining and improving service quality standards.

Schedule
Monday to Sunday on a rota basis
5 days on 3 days off.
Hours to be covered are 5am to 2pm or 2pm to 11pm
