Job summary
We are seeking a motivated Band 6 Respiratory Specialist Nurse to support delivery of high quality, patient centered care across acute and community pathways. The role will provide expert assessment and management for patients with complex respiratory conditions, contributing to admission avoidance and reduced length of stay. You will work closely with the multidisciplinary team to improve patient flow, enhance clinical outcomes, and support ongoing service development.
Main duties of the job
- Demonstrate empathetic interpersonal and communication skills in dealingwith patients and carers through the patient pathway.
- Demonstrate advanced practice in holistic assessment, planning,implementation and evaluation of care within the specialism. This willnecessitate high levels of autonomy and advocacy and include receivingdirect referrals and ensuring the delivery of appropriate care for a caseloadof patients.
- Demonstrate advanced knowledge, skills and innovative practice inrelevant specialist area. Ensure that there is a system of nursingdocumentation, which reflects this advanced practice. Promote clinicalexcellence reflecting the Sandwell & West Birmingham Hospitals NHSTrust nursing strategy and business plans.
- The post holder will aim to prevent hospital admissions and reduce lengthof stay, when possible, through providing treatment and support to thosewith long term respiratory conditions in line with national and localguidelines
