Job summary
We are looking for two Band 6 Community Nurses to join our East Riding CORE Team, based at Rivendell House, Driffield, and our Hull CORE Team, based at West End.
In this role, you will deliver mental health care to children, young people, and families within a community-based setting. Working as part of a multidisciplinary team, you will provide assessment, support, and treatment for individuals experiencing emotional and psychological difficulties.

Main duties of the job
- Conduct comprehensive family partnership and mental health assessments for children and adolescents presenting with a range of emotional, behavioural, and psychological issues.
- Develop individualised care plans, ensuring the involvement of children, families, and other relevant professionals.
- Use clinical judgement and evidence-based practices to determine appropriate interventions.
- Offer direct clinical care, and ensure the monitoring and evaluation of treatment progress. Linking in with other parts of the wider CAMHS network when necessary such as crisis and home treatment team, this will include multi agency working and systemic working within the young persons network.

Qualifications and knowledge
Essential
- Degree or diploma in Adult Nursing
- NMC Registration
- Full understanding of the practice area and specific role applied for demonstrated on application.
Desirable
- Evidence of CPD or experiential learning at an advanced level e.g. Postgraduate/Masters level.
Experience
Essential
- Previous community based experience or band 6 experience transferrable to a community setting
- Evidence of development of specialist practice nursing skills and able to demonstrate the impact of this on practice change/development
Desirable
- Demonstrable experience working in community nursing at band 6 level
- Experience of SystmOne
- Leadership/management experience which has had a positive impact and created change within the service delivery/practice.
Skills and Competencies
Essential
- Ability to effectively transport self between multiple addresses/venues on a daily basis including transporting equipment & clinical stock required for treatment.
