Job summary
The Older Adults Duty and Triage Team have an opportunity for a full-time band 6 Registered Community Mental Health Nurse or Occupational Therapist (advertised separately).
The Team works predominately with people aged over 65 who have dementia or complex mental health problems; the team covers all of Worcestershire.
The post involves working as a cohesive team to manage and triage the incoming referrals to the service, either signposting to other agencies or if appropriate assigning them to a community mental health team for assessment and treatment. The post also involves visiting patients that you have identified as requiring urgent input to complete an initial assessment and arrange appropriate follow up.
You will need to have experience with the patient group and community work, with a sound knowledge of risk assessment; you will be offered 1-1 supervision and ongoing support. You will enjoy working in a close-knit team and supportive team.
The 37.5 hours are to be worked Monday to Friday 9.00am to 5.00pm, although the team is flexible and supports work-life balance. For the right candidate we can offer part time hours at a minimum of 22.5 hours per week.
Main duties of the job
Undertaking urgent assessments of mental health in various settings in the community to include risk assessment
Triaging paper referrals to include telephone triage to determine the most appropriate service for ongoing mental health support
Accessing various data bases to seek information to include Care Notes, ICE and Adult Social Care records. Recording on our Care Notes data base
Professional telephone conversations with partner agencies, patients, relatives, carers
Arranging admissions to hospital where any assessment you have undertaken requires this, this can include liaison with the Approved Mental Health Professional Service for anyone requiring admission under the Mental Health Act.
Liaison with the Community Area Consultant Psychiatrists as required
Liaison and referral, to include joint visits where indicated, with our Hospital at Home Team
Attending professionals meetings
The role does not involve holding patient cases for any prolonged intervention.
