We are seeking a motivated and enthusiastic Band 6 Diabetes Specialist Nurse to join the North Middlesex Diabetes Team and support the delivery of high-quality, patient-centred diabetes care across the Trust.
The post holder will work across inpatient and outpatient settings, supporting the assessment, management and education of adults with diabetes, including those with complex needs. Working alongside senior Diabetes Specialist Nurses and the wider multidisciplinary team, you will contribute to safe and effective diabetes management, helping to improve glycaemic control and reduce diabetes-related complications.
You will provide specialist nursing input to ward teams, supporting insulin management and the recognition and management of hypoglycaemia and hyperglycaemia, in line with local and national guidance. Patient education and staff training are key aspects of the role, promoting best practice in insulin safety, sick day rules and diabetes self-management.
The role involves close collaboration with diabetes consultants, dietitians, podiatrists, pharmacists, and community and primary care colleagues, as well as participation in audit, service development and quality improvement activities.

Main duties of the job
Support the delivery of high-quality, evidence-based diabetes care to adults across inpatient and outpatient services at North Middlesex University Hospital. Work as part of the multidisciplinary diabetes team to assess, plan, implement and evaluate individualised care for patients with type 1, type 2 and secondary diabetes. Support senior Diabetes Specialist Nurses and diabetes consultants in the management of complex diabetes cases, including insulin initiation, titration and ongoing glycaemic monitoring. Provide timely specialist advice and clinical support to ward teams across medical and surgical areas to promote safe and effective diabetes management. Support the recognition and management of hypoglycaemia, hyperglycaemia and diabetes-related complications in line with Trust policies and national guidance. Promote insulin safety and reduce diabetes-related harm through education, clinical oversight and appropriate escalation of concerns. Deliver structured patient education to enable individuals to develop the knowledge, skills and confidence required to self-manage their diabetes effectively. Contribute to education and training for nursing staff, junior doctors and allied health professionals, promoting best practice in diabetes care. Work collaboratively with dietitians, podiatrists, pharmacists, community teams and primary care colleagues to support integrated care pathways.
