Job summary

We have an exciting opportunity for a Diabetes Specialist Nurse to join our team at Shropshire Community Health Trust. The purpose of the role is to be part of providing a delivery of a comprehensive, skilled and evidence based Diabetes Service to meet the needs of people with diabetes.

The role will provide leadership skills, management and development within the Diabetes Team. Maintain a clinical caseload within your specialist field. Continuing responsibility for assessment of health needs and development and evaluation of standards of care.

Main duties of the job

  • To assess, plan and implement and evaluate evidence-based nursing care for people with diabetes.
  • The post holder is required to provide expert clinical care for patients and to support other staff to develop their competencies in diabetes management.
  • The post holder will be involved in teaching and training both formally to clinical staff and to patients.
  • The post holder will be required to work collaboratively across primary and community care, and link to other specialists within the acute setting to implement the integrated care pathway for people with diabetes.

Person Specification

Training and Qualifications

Essential

  • Registered Nurse/RGN
  • Evidence of gaining post registration CPD or experience in diabetes nursing
  • Developed specialist knowledge through working with patients with chronic conditions including diabetes
  • First Level Degree or equivalent
  • Recognised Diabetes Qualification or currently undertaking or willing to undertake. This qualification must be obtained within 2 years of appointment and will be supported by the Trust
  • Teaching Certificate or proven experience of providing training/education to a range of staff and patients
  • Evidence of continual professional development and education

Experience

Essential

  • Post registration experience
  • Evidence of nursing people with diabetes in an acute or community care setting
  • Experience of provision of training/education to range of staff and patients

Knowledge and Skills

Essential

  • Advanced communication, negotiation, decision making and organisational skills
  • Ability to communicate complex and distressing information to patients and their carers in a sensitive & supportive way at a level they can understand
  • Ability to manage complex caseload of high risk patients
  • Advanced training and utilisation skills in therapeutic/diagnosis procedures & equipment relevant to diabetes
  • IT skills and willingness to advance
