Job summary
The Practice Nurse will work as part of the Chapel Street Enhanced Care Home Team at Brierley Hill Health and Social Care Centre under the management of the Trust to provide nursing care to the patients in residential and nursing care settings within the community in accordance with NMC (Nursing and Midwifery Council) code of conduct and in line with the NMC scope of professional conduct.
Chronic Disease Management would be an advantage but Older Adult Vaccinations, Cervical Screening, Wound Management and an understanding of QOF are essential requirements for this role.
The successful candidate will have the ability to work independently as well as being a good team player. The ability to work under pressure is essential.
As an EMIS Web organisation previous experience using this system would be desirable but not essential as full training would be given.
Main duties of the job
Duties and Responsibilities:
- Assist in and perform routine tasks related to patient care as directed by Nurse Lead and GPs
- Cytology
Venepuncture
Adult Vaccinations
- Requesting pathology tests, for example urine culture, swabs
- LTC management including Diabetes, Coronary Heart Disease, Hypertension, Asthma, CVA, COPD
- Ordering of vaccinations to maintain stock levels
- Ability to deliver COVID Vaccinations
- Following agreed clinical protocols with referral from GPs as appropriate
- Awareness of infection control and requirement to take part in infection control audit
- Participation in administration systems in the Practice (EMIS, CMCS, ICE, Docman)
- At all times there will be a need to maintain accurate records
- A duty to advise GPs of potential problems or errors within a range of assigned tasks
- Attend and participate in any Nurse and Practice meetings when required
- Any other delegated duties appropriate to the post
