Job summary
Plymouth home mechanical ventilation team provide a serviceacross theSouth WestPeninsula.Itcurrently provides a home ventilation service for over500 patients living in Devon and Cornwall.The service alsoprovides aspecialistservice to patients with long term tracheostomyboth at home and within the hospital environment. Inadditionitprovidesan oxygen assessment and review service to patients within the Derriford Footprint.
As a senior and highly specialist practitionerwithin thisteamthe post holderwillprovideexpert leadership and clinical advice to staffof all grades and professions,patientsand theircarers.
Inaddition the post holder will lead the weak cough service provided by the team.
***Preference will be given to internal Trust staff, as well as 'Priority' and 'At Risk' status including NHS At Risk staff throughout Devon who are able to display recent relevant experience as dictated by the Person Specification.***
Main duties of the job

Within the context of a multidisciplinary team, using higher levels of decision making and clinical judgementtobe anautonomousexpert practitioner for adults and transitioning young people withchronic ventilatory, oxygen or cough failure or those likely to develop these conditions.

To support and adviseother subspecialist services within respiratory medicine whereadults and transitioning young people require home mechanical ventilation, coughaugmentationor home oxygen therapy.

To act as an expertclinical resource, and facilitator to educate andtrainmedical,nursingand allied health professionals asrequired.

To support the development of thelongtermdomiciliaryventilationservice according to need,undertakingauditandresearchprojects and develop policies and procedures asrequired.

To leadthe weak cough service provided by the home ventilation team.

Work autonomously to manage own patient caseload, this includesundertaking non-medical clinics,domiciliaryvisitsand outreachinginto other hospitals.

Autonomously undertakes comprehensive assessments of respiratory needs of new and established patients with ventilatory, oxygen andcoughfailureincluding the need for airway clearance techniques.

Communicate highlysensitive,complexand emotive information to patients and relatives independently, without a doctor present, with empathy and reassurance. This willincludingissues such as diagnosis, diseaseprogressionand end of life, or explaining treatment options and aiding decision making.

Completion of treatment escalation plansand allow a natural deathforms.

Autonomouslyorder,undertakeand interpretinpatient and outpatient investigations including those specific to patients accessing this service. These includephlebotomy, X-rays,radialartery puncture andblood gas analysis,capnographyandoximetry. Change managementautonomouslydepending on results.This will include prescribing medicines.

Initiate drug therapy/medication and other changes in patient management including commencing anddiscontinuinglong term ventilation andin accordance withpresent legislation supply and prescribe medicines independently as a non-medical prescriber.

Autonomouslyinitiatenon-invasive ventilation. augmented cough techniques and oxygentherapy forpatients who need these treatments as inpatients, outpatients or in thedomiciliarysettings.

Autonomously develop andimplement managementand treatment plans for patientsthroughouttheir disease trajectory wherever they maybe: inpatient,outpatient, at home, whilst outreaching into other hospitals.

Develop specialised programmes of care andprovidehighly specialised advice including written and oral information to other professionals including senior medical staff, patients, theirrelativesand cares.
