An opportunity has arisen for an experienced Lead Advanced Practitioner to join the East Coast Community Healthcare Primary Care Homes team, leading the expansion of Urgent Community Response. East Coast Community Healthcare are committed to promoting the development of Advanced Practice in our Primary Care Homes and are seeking a Lead Advanced Practitioner as part of this commitment. The successful applicant will drive the development of robust clinical pathways; enabling urgent assessment and intervention for higher acuity clinical need in the community. The post holder will work with our multi professional Advanced Practitioners to fully embed the Advanced Practice role and comprehensively enhance the urgent response clinical skills of our community teams. As a key aspect of this role, the successful candidate will work in collaboration with lead clinicians in partner organisations to fully engage in the sharing of best practice and strive for equity of service provision across the region.

Main duties of the job

East Coast Community Healthcare (ECCH) works collaboratively with partner organisations to deliver Urgent Community Response closer to home. Advanced Practitioners play a critical role within this pathway. Characterised by a high degree of autonomy and designated responsibility for complex decision making the post holder will encompass all four pillars of clinical practice and will have highly advanced skills in the analysis and synthesis of complex problems, management of clinical risk.

The Lead Advanced Practitioner will provide robust clinical leadership within the rapidly evolving UCR and virtual ward space. Key objectives for the role will be:

* The development and communication of clearly defined clinical pathways compliant with national clinical guidelines

* The development of robust governance processes which provide assurance of patient safety across UCR within the ECCH geographical area

* Supporting and actively promoting the development of APS within the service; recognising and respecting professional variation

The successful applicant must have completed an MSc in Advanced Practice (or equivalent subject to module scrutiny) and have consolidated fully academically and clinically.

The successful applicant will be responsible for the clinical leadership and strategic clinical development, of all ECCH Advanced Practitioners aligned to UCR pathways. This will include but not be limited to Advanced Practitioners aligned to East Hospital at Home (virtual ward).

The Primary Care Homes are multi-professional community teams, capable of flexing to meet both the planned and unplanned needs of the local population. The PCH teams are multi-professional, including Nurses, Occupational Therapists, Physiotherapists, Paramedics, and Pharmacy Technicians, as well as a wide range of trainee and support roles. Urgent Community Response is delivered 24 hours per day, 7 days a week. East Coast Community Healthcare are committed to the development of Advanced Practice within our Primary Care Homes.

Job responsibilities

The post holder responsibilities will include:

*The Strategic Clinical Development and Clinical Leadership of ACP's (and equivalent roles) within Urgent Community Response pathways including East Hospital at Home (virtual ward).

*Supporting the delivery of an approved Advanced Practice curriculum (national or local in absence) for trainee practitioners.

*Contributing to system wide strategies surrounding performance and improvement.

*Contributing to workforce planning and considerations around organisational systems affecting resilience and retention.

*Developing and embedding a robust governance structure to the training and development of Advanced Practice across ECCH Primary Care Homes.

*Supporting the management of budgets for Advanced Practice in the community.

*Developing robust clinical pathways in close partnership with acute, community and senior manager colleagues, aimed at safe patient care, experience and efficiency.

*Line management of PCH Advanced Practitioners and Trainee Advanced Practitioners

Communication and Key Working Relationships

*To communicate in a highly skilled and sensitive manner, highly technical, clinically sensitive /or condition-related information to clients, their families and other healthcare professionals both verbally and in written format.

*Communicate effectively with individuals, their families, carers and health/social care practitioners using a range of styles and ensuring information always remains confidential.

*Proactive in seeking out different styles and methods of communication to assist longer term needs and aims.

*Engage fully and contribute in communication and negotiation regarding service provision for East Hospital at Home (virtual ward) and Urgent Community Response, considering how Advanced Practice can support progress and evolution of the service.

*Collaborate and communicate effectively with other specialties, clinical areas, disciplines and services to deliver high standards of efficient care to your patients. Utilise effective communication techniques to integrate into the multidisciplinary team.

Planning and Organisation

*Responsible for the development and delivery of pathways and processes that deliver the key performance indicators for East Hospital at Home? (virtual ward) and Urgent Community Response.

*Working in close partnership with the PCH Operations Manager to actively shape the direction of East Hospital and Home (virtual ward) and Urgent Community Response; to deliver the performance targets and service specifications.

*Ensure the ECCH Advanced Practice Strategy is embedded into Urgent Community Response including East Hospital at Home (virtual ward).

*Participate in appropriate system wide and organisational committees, working parties and project teams, leading on the delivery of high clinical standards.

*Support and contribute to the design, alongside colleagues, of new, innovative integrated services that meet national quality indicators and targets.

*Be willing to represent ECCH at local and national level forums with regards to Advanced Practice.

Analytics

*To use advanced analytical and judgement skills to support the assessment of patients in highly complex situations; integrate information from multiple levels in diagnoses.

*To undertake advanced and comprehensive assessment, investigations and diagnoses based on a body of specialist knowledge.

*Continue provision of care in a determined care plan on the already differentiated and diagnosed patients or adapt care relevant to further assessment, investigations or differentiation/diagnosis in accordance with governance protocols relevant to area of working.

*Responsible for ensuring that information systems adopted by ECCH or partner organisations are implemented and maintained in their service area, that their service uses such systems effectively and that the information available is analysed and evaluated, to manage performance and to meet ECCH and system objectives.

*Lead on the use of advanced analytic approaches including the use of point of care testing to inform clinical decision making and care planning.

Responsibility for Patient / Client Care, Treatment & Therapy

*Deliver and participate in high standards of clinical care delivered to the patient within core operational hours

*To anticipate and plan for emerging needs outside core operational hours

*To be highly visible to all staff, patients and carers to be able to respond to requests for specialist assistance, advice and support, particularly concerning provision of clinical care.

*Scrutinise patient care plans and address any issues which may result in poorer patient outcomes

*To manage a proportionate caseload of patients autonomously and independently in accordance with governance protocols relevant to area of working.

*To support the management of a shared caseload of highly complex patients across a 7-day working pattern, alongside other advanced practitioners.

*Ensure all aspects of practice are undertaken with obtained informed consent where applicable, in accordance with local policy, professional body guidance and law.

*Be directly responsible for and manage patients as part of their own caseload.

*Contribute and assist in safe and efficient patient flow and become involved in potential areas of improvement within this area.

*Be directly involved in patient experience quality assessment and implementation of improvement strategies.

Policy, Service, Research & Development Responsibility

*Responsible for interpreting, communicating and implementing ECCH policies working closely with the PCH Operations Manager, and to participate in the proposal and development of new policies related to Urgent Community Response and East Hospital at Home (virtual ward).

*Implement organisational change within the service and ensure change is effectively project managed adhering to ECCH policies.

*Responsible for interpreting national policies, guidelines and recommendations to facilitate a standardised approach to Urgent Community Response and the Advanced Practice role within East Hospital at Home (virtual ward).

*Responsible for the development and implementation for clinical protocols and polices for the service.

*To be fully aware of the Safeguarding policy and implementation requirements acting as a resource for other team members as necessary.

*Adhere to ECCH policies and procedures particularly in relation to confidentiality, health and safety and financial instructions and ensure that local policies and procedures are implemented in relation to the post and the team.

*Ensure accurate and legible documentation and records are maintained in accordance with the Data Protection Act 1998.

*Practice in accordance with specific professional body codes of professional behaviour.

*Understand all relevant trust policy and guidelines and implement them correctly.

*Working knowledge and experience of patient safety. Ability to identify trends and themes, identify problems and lead on lessons learnt; making recommendations to improve patient safety and prevent patient harm. Responsible for managing the investigation process to meet national reporting standards, such as Duty of Candour.

Responsibility for Finance, Equipment & Other Resources

*Responsible in understanding budget management within health.

*Regularly monitors financial information in line with the appropriate funding for the service.

*Be able to reconcile contrasting views and deliver balanced arguments, considering current budget restraints.

*Be able to apply financial scrutiny and evaluation to service change and development strategies.

*Be involved in and comply with equipment management used and maintained within the clinical area.

*Ensure any trust issued IT equipment is maintained and kept safely and securely in accordance with IT policy.

Responsibility for Supervision, Leadership & Management

*Support recruitment across the service by working with the Clinical and Operational teams to agree appropriate workforce levels and skill mix in accordance with budget; participating in recruitment panels where required and supporting the selection process for staff.

*Line manage PCH Advanced Practitioners and Trainee Advanced Practitioners, including approving leave, undertaking appraisals and managing performance

*Act as a role model working with a high level of personal standards and self-discipline whilst delivering a high standard of care to your patients.

*Be an approachable senior clinical leader and support junior staff in their own development and learning.

*Supervise, mentor and act as a resource to trainee Clinical Practitioners across the directorate.

*Be responsible for maintaining their own professional development above and beyond that expected of their MSc.

*Ensure you maintain qualifications in relevant national courses for your role and if an instructor, continue to engage in these activities of education.

*If not already completed, be willing to complete specialty credential or demonstrate clinical portfolio to an equivalent from the relevant Royal College if such a curriculum is not yet in existence.

*To support the education and training of the team on aspects of quality including incident management, governance, complaints management, audit, CQC and Patient Safety compliance.

*Ensure that lessons from internal and external incidents, claims, complaints and inquest verdict are learned and shared so that timely feedback of information occurs.

• *Evidence of level 8 study or willing to undertake in the future.
