An exciting opportunity has arisen for a Band 6 Registered Mental Health Nurse to join urgent care services at the Salford Home Based Treatment.
Salford Home Based Treatment Team is a 24/7 service that supports individuals in crisis at home, as an alternative to psychiatric inpatient admission.
As a key member of a multidisciplinary team, you will undertake comprehensive mental health assessments, develop collaborative care plans, provide evidence based interventions, undertake risk assessment and managment, and support service users and families during periods of significant distress.
You will work closely with inpatient services, community teams, emergency services, and partner agencies to ensure safe, effective, and recovery focused care.
We are looking for applicants who demonstrate excellent assessment and communication skills, sound clinical judgment and a commitment to delivering person-centred, recovery focused care.
The team operates a 24/7 service, therefore there will be a requirement to cover shift work including nights.

To effectively screen and gatekeep referrals to urgent care services, ensuring that the least restrictive options for treatment are offered

To provide comprehensive assessment of a service users mental state and social needs which would aid the completion of a collaborative care plan for community management.

To provide comprehensive risk assessment, formulation and management as part of the service user's journey.

To act as keyworker for service users with support from the multi-disciplinary team.

Cover the duty / shift coordination role at times.

Liaison with other agencies involved in a service users care.

Lead on the discharge or transfer of care for our service users to appropriate agencies.

To support and supervise junior members of staff.
