Job responsibilities
Main Duties and Responsibilities
- Work autonomously as an Advanced Clinical Practitioner (ACP) within the Neighbourhood Team, managing a complex caseload and prioritising clinical activity in line with agreed pathways, protocols and service objectives.
- Deliver high-quality, person-centred care, taking responsibility for the advanced assessment, examination, diagnosis, investigation and management of patients presenting with a wide range of undifferentiated and undiagnosed conditions within the community setting.
- Exercise expert clinical judgement and decision-making to develop, implement and evaluate individualised care plans that promote safe, effective and timely care.
- Provide appropriate treatment and interventions, ensuring patients are managed safely and effectively, with onward referral, escalation or discharge arranged as clinically appropriate.
- Receive, review and triage referrals from a variety of sources, including healthcare professionals, partner agencies and direct patient referrals, ensuring patients are directed to the most appropriate service and care pathway.
- Participate in multidisciplinary team meetings, patient reviews and case discussions, contributing specialist clinical expertise to support collaborative decision-making and integrated patient care.
- Utilise the full scope of advanced clinical practice, including Independent Non-Medical Prescribing, providing expert advice on medicines optimisation and management in accordance with Trust policies, professional standards and best practice guidance.
- Act as a clinical role model, supporting service development, quality improvement and the delivery of safe, responsive and innovative community-based care.

Education
Essential
- Current registration with NMC, HCPC or GPhC
- Non- Medical prescribing course (in eligible professions)
- Master's Degree in Advanced Clinical Practice.
- Evidence of continuing professional development relevant to advanced clinical practice.

- Teaching or relevant experience qualification
