Job summary

An exciting opportunity has arisen for an enthusiastic, well-motivated and flexible Nurse Practitioner to join our Urgent Community Response Services across Sunderland within the Recovery at Home. The team consists of an extensive MDT which includes Nurse Practitioners, Registered Nurses, Occupational Therapists, Physiotherapists, Health Care Assistants, Exercise Practitioners and Administrative staff. The team also works closely alongside partner agencies including those from the local and voluntary sector.

This role would involve assessing people in their own homes who are either acutely unwell or have recently been discharged from hospital and require enhanced intervention and short-term support for their physical and functional health needs to remain at home. The service is fast paced, and you would be expected to see a wide range of patients who present with a variety of conditions. This is a unique and rewarding role which will allow you to work with patients within their usual place of residence to enable them to remain there safely alongside you developing your own clinical skills.

The post holder will need to be proactive with their own learning and work to identified timescales.

You will possess the following:

1st level Nurse with significant post registration experience.

Nursing Degree. Non-Medical Prescribing qualification (or due to complete within next 6 months).

Portfolio of evidence of competence and continuous professional development.

Leadership qualification/training.

A FULL DRIVING LICENCE IS AN ESSENTIAL REQUIREMENT OF THIS POST.

  • The post holder will take a lead role in The Recovery at Home Team, providing care to people in their own homes to prevent admission/readmission to hospital and also supporting early discharge from hospital by providing rapid assessment of patients in the community over a 24 hour period.

  • To work autonomously, assessing individual needs, initiating investigations, determining a differential diagnosis and initiating appropriate holistic, evidence based treatment and care. Referpatients who require medical intervention to the appropriate services.

  • Undertake triage within the service determining clinical priority of calls working with the coordinator and GP ensuring teams are dispatched to respond to the referrals.

  • To prioritise and delegate work appropriately monitoring and ensuring safe working practice.

  • Significant experience of working at a senior level caring for the acutely unwell/deteriorating patient
