We have an exciting developmental opportunity to enhance your knowledge and skills regarding complex discharge planning.As a member of the Specialist Discharge Team, you will be a case manager alongside the wider MDT to plan complex discharges. You will be expected to speak with patients, their relatives, and all members of the MDT, as well as external partners to ensure a safe, timely discharge; therefore, excellent communication skills both face to face, by telephone and email are essential.
The ability to work efficiently and proactively in a fast-paced environment to improve patient experience and patient flow is essential. Working in a supportive team you will have good problem solving and decision-making skills.The role requires an eye for detail and excellent IT skills ideally with a particular proficiency in digital patient records and Microsoft Office Suite.
The successful candidate will be based at either the Royal Victoria Infirmary or Freeman Hospital site but must be flexible to work across either site as required. Currently the role is Monday to Friday between the hours of 8am and 4pm, but we are looking at a potential move to seven days working once fully staffed.

Main duties of the job
You will be the case manager for a group of complex patients and provide specialist discharge advice and support to specific multidisciplinary teams.
You will have daily contact with patients, families, and staff to proactively plan and facilitate the safe and timely discharge of patients with complex discharge care plans.
You will be trained to use our data platform OPTICA and ensure that up to date information regarding each patient's journey through the system is available daily and ensure that any problems which may impede the speed, safety and effective discharge of a patient are identified as early as possible.

- Diploma plus specialist qualifications / experience within specialist area; prepared to take degree level study
- Meets Nursing and Midwifery Council standards for student supervision and assessment (formally Mentorship) if relevant to role.
- Current driving licence if relevant to role

- Degree in health related subject

- Some post registration experience within related specialist area of practice
- Extended clinical practice underpinned by training supervision & assessment.
