Job summary

An exciting opportunity has become available within the Adult Health and Social Care Community Therapy Team.This innovative post provides the opportunity to work as part of in integrated Independent Living Service in Salford providing assessments of peoples in a variety of community settings.

You will be responsible for your own caseload and have the opportunity to work closely with the wider community MDTs, adopting an integrated approach to patient care across Salford.

You will be a motivated OT with excellent communication skills, enthusiasm and commitment to the team.

We are looking for a strong approach to team working and a drive for service development.

Potential TUPE Transfer

Adult Social Care services in Salford are currently delivered by Northern Care Alliance NHS Foundation Trust and are expected to transfer to Salford City Council during 2026.

As this role forms part of those services, the successful candidate is likely to transfer to Salford City Council under the Transfer of Undertakings (Protection of Employment) Regulations 2006 (TUPE), subject to the transfer proceeding as planned. Any transfer would be managed through the appropriate formal process, including consultation and support for affected employees.

Main duties of the job

  • To triage and prioritise referrals into the service
  • To carry out holistic assessments of the functional abilities of people in the community whose needs can be complex, changing, and varied across the caseload.
  • To assess, plan, prioritise and provide outcome solutions including equipment, and minor and major home adaptations to a community-based client group.
  • To plan, organise and manage a community caseload with supervision.
  • To contribute to the service development of community-based therapy services.
  • To provide professional guidance and support to Community Assessment Officers and/or students

Registration.

Essential

  • HCPC Registration.
  • RCOT Membership

Essential Qualifications.

  • Degree in Occupational Therapy.
  • Full UK driving Licence.

Knowledge,Skills,Training and Experience.

Essential

  • Good understanding of both physical and mental health to provide a holistic approach.
  • Good understanding of the role of an Occupational Therapist
  • Good Understanding of Risk Assessment.
  • Understanding of the management of long term conditions in the home environment.
  • Aware of the roles of the MDT
  • Aware of the limitations of Occupational Therapy.
  • Able to work as a team member, manage time and caseload.
  • Able to work under pressure and flexibly.
  • Good understanding of Governance.
  • Use of a car or access to means of travel across the Trust Footprint.

Desirable

  • Experience of clinical report writing.
  • Formal presentation Skills.
