We are excited to announce an opportunity for a Band 7 Senior Practitioner to join the Rough Sleepers Mental Health Project (RAMHP) in the London Borough of Hackney. This position is a one-year fixed-term contract.
We are seeking a Mental Health Professional--whether a Mental Health Nurse, Mental Health Social Worker, or Occupational Therapist--who is a trained Approved Mental Health Professional (AMHP) to become part of the RAMHP team and work throughout Hackney. This role focuses on supporting a complex and vulnerable group of service users.
Candidates should have a solid understanding of the health needs of rough sleepers and the numerous challenges that homeless individuals face in accessing health services. We are looking for an experienced clinician with outstanding communication skills, advanced assessment abilities, and a robust clinical background, all of which will enable effective collaboration with service users as well as statutory and partner organizations that assist people in difficult situations.
Main duties of the job
The team uses an assertive outreach approach, going out to see people where they are. We are looking for people who are trauma informed and creative in how they engage and support people with multiple needs.
Practitioners need expertise in bio-psychosocial assessment and liaison. The practitioner will be experienced in safeguarding adults and have a good working knowledge of relevant legislation including the mental health act, mental capacity act and the Care Act.
The Practitioner will be an active member of the multidisciplinary team, provide bio-psychosocial assessments and support to rough sleepers referred by the street outreach teams. Trained AMHPs will contribute to the Hackney AMHP rota.

Job responsibilities
- The post holder will be an active member of the multi-disciplinary team providing care and support to rough sleepers referred to the service by the street outreach teams.
- To manage a complex client caseload as defined by the ELFT Rough Sleepers Mental Health Project (RAMHP)
- Along with the Operational Lead, to provide day to day operational management of the Rough Sleepers Mental Health Project.
- The service offers a team approach applying concepts of recovery to service delivery. Therefore advanced clinical reasoning for assessment, therapeutic interventions and management skills will be required, together with the provision of wide ranging practical support and assistance that will promote and maintain the quality of life of service users and rough sleepers
- The post holder will establish and maintain strong working relationships with Street Outreach Teams, Local Authority Rough Sleepers Services, third sector organisations, Local Authority Adult Social Care, Housing Needs Department, CMHTs, acute services, Local authorities, GPs and day centres, employment and training providers in order to meet the individual needs the service users. Therefore good communication and negotiation skills will be required as well as an understanding and experience of partnership working.
- There is a clear responsibility of the Senior Practitioner to act as primary accountable lead for communication with other agencies, even after admission or a service user to the inpatient unit. Furthermore the post holder will monitor this practice for staff under their supervision.

- Ensures that the voice of service users / rough sleepers are captured and listened to.
- Facilitates regular time and space for team reflection
- Monitors quality and performance of the service through tracking key measures over time, supporting the use of transparent and visual management systems
- Involves a wide range of stakeholders and data/information in helping the team identify their key improvement priorities.
- Along with the Operational Lead develop the work with the rough sleeping population by participating in the training of colleagues across statutory and voluntary sectors.

Education/ Qualification/ Training
Essential
- Relevant clinical qualification (RMN, Social Worker, OT)
- AMHP trained
- Active registration with relevant clinical body Evidence of post qualification clinical training, regular updating and ongoing development of role and skills
- The post holder will be required to have short specialist courses to Masters Level or equivalent relevant to the post
- Evidence of continual professional development
- Higher Education qualification in Community Mental Health
- Mentorship Training or equivalent
- Recognized post graduate qualifications in care of the adult with mental health problems
Experience

- Experience of working in a multi - professional team
- Understanding of the skills needed to supervise members of a multi-disciplinary team plus experience of supervising professional staff (this might include trainees)
