Job summary

We are seeking a motivated and compassionate Band 5 Registered Nurse to join our Stroke Rehabilitation Unit. This is an exciting opportunity to be part of a dynamic multidisciplinary team delivering high-quality, patient-centred care to individuals recovering from stroke.

The successful candidate will play a key role in supporting patients through their rehabilitation journey, promoting independence, and maximising functional recovery. You will be responsible for assessing, planning, implementing, and evaluating care, while working collaboratively with physiotherapists, occupational therapists, speech and language therapists, and medical staff.

This role requires excellent clinical skills, strong communication, and a commitment to delivering evidence-based practice. You will be supported with ongoing professional development opportunities and encouraged to enhance your knowledge in stroke care and rehabilitation.

Main duties of the job

The post holder will be responsible for delivering high standards of nursing care to patients undergoing stroke rehabilitation, ensuring care is safe, effective, and patient-centred at all times. Key duties include:

  • Assess, plan, implement, and evaluate individualised care plans for patients with complex physical, cognitive, and emotional needs following stroke.
  • Support patients in achieving their rehabilitation goals by promoting independence and encouraging participation in daily activities.
  • Work collaboratively within the MDT to coordinate care and ensure a holistic approach to recovery.
  • Administer medications and treatments in accordance with Trust policies and the Nursing and Midwifery Council (NMC) Code.
  • Monitor and respond to changes in patients' clinical conditions, escalating concerns appropriately.
  • Provide education, advice, and emotional support to patients, families, and carers throughout the rehabilitation process.
  • Maintain accurate, timely, and comprehensive patient records in line with legal and professional standards.
  • Participate in discharge planning to ensure safe and effective transitions of care.
  • Contribute to maintaining a safe clinical environment, including infection prevention and control and safeguarding responsibilities.
  • Act as a role model for junior staff and support students/newly qualified nurses where appropriate.
  • Engage in continuing professional development and actively participate in training, supervision, and appraisal processes.

Clinical Responsibilities

  • Deliver safe, effective, and compassionate nursing care in accordance with the Nursing and Midwifery Council (NMC) Code and local Trust policies.
  • Undertake comprehensive assessments of patients physical, psychological, and social needs, including risks associated with reduced mobility, swallowing difficulties, and cognitive impairment.
  • Develop, implement, and regularly evaluate individualised care and rehabilitation plans in partnership with patients and the multidisciplinary team.
  • Support and carry out stroke-specific rehabilitation interventions, including mobility support, pressure area care, continence management, and nutritional support.
  • Safely administer medications and monitor their effects, ensuring timely escalation of concerns where required.
  • Recognise and respond promptly to any deterioration in a patients condition, initiating appropriate interventions and escalation.

Multidisciplinary Team Working

  • Work collaboratively with physiotherapists, occupational therapists, speech and language therapists, doctors, and other healthcare professionals to deliver coordinated care.
  • Participate in multidisciplinary team meetings, case discussions, and goal-setting processes.
  • Contribute to discharge planning, ensuring safe, timely, and well-coordinated transitions of care.

Communication and Patient Support

  • Communicate effectively with patients who may have communication or cognitive difficulties, adapting approaches as needed.
  • Provide education and support to patients, relatives, and carers regarding stroke recovery, rehabilitation goals, and ongoing care needs.
  • Promote dignity, respect, and patient choice at all times.
