Job summary

Are you looking for a rewarding rotational role in the community, supporting patients across Hull and the East Riding to regain confidence, independence and quality of life? We are pleased to offer an exciting rotational post across Hull and East Riding Community Rehabilitation and Hull and East Riding Intermediate Care. This is an excellent opportunity to develop your skills across varied community settings while making a meaningful difference to patients following illness, injury or a change in their condition. This role helps people regain independence at home by providing practical support and rehabilitation, making a real difference to everyday life. You'll work as part of a supportive Community Rehabilitation team, helping adults with a range of physical and cognitive needs. You'll assess, plan and deliver care that promotes choice, safety and independence. Your work will support people and carers to build confidence and manage daily activities.

Main duties of the job

What you'll do:

  • Assess people's needs and plan personalised occupational therapy interventions
  • Deliver treatment programmes in homes and community settings
  • Support people to use equipment safely and adapt to daily living tasks
  • Manage and prioritise your own caseload to meet changing needs
  • Work closely with colleagues, carers and partner services

What we're looking for:

  • A degree or diploma in Occupational Therapy and Health and Care Professions Council (HCPC) registration
  • Understanding of occupational therapy practice and community-based care
  • Good communication skills, including supporting people with complex needs
  • Ability to manage a varied workload and travel between locations
  • Willingness to learn through preceptorship and ongoing development

Qualifications

Essential

  • Diploma or Degree in Occupational Therapy
  • Current HCPC Registration

Desirable

  • Demonstrable experience as a Practice Educator of preregistration students
  • Evidence of current CPD/formal qualifications at an advanced level e.g. degree or post graduate level or equivalent knowledge/experiential learning

Knowledge & Experience

Essential

  • A broad understanding of relevant policy, legislation drivers and their application to clinical and service area, e.g. Mental Capacity Act, Social Inclusion
  • Knowledge of Occupational Therapy models of practice, standardised assessments and their use
  • Basic IT skills
  • Willingness to undertake and complete preceptorship
  • Appropriate knowledge/experience of working in community and adult physical health clinical practice areas
  • Understanding of caseload management and responsibilities
  • Conversant in the 7 C's - Care, Compassion, Competence, Communication, Courage, Commitment and Candour and the values of the organisation
  • Able to demonstrate effective inter-personal/communication skills
  • Evidence of good time management skills

Desirable

  • Full understanding of relevant policy, drivers and their application to clinical and service area, e.g. NSF, Social Inclusion
  • Full understanding/application of relevant clinical practice/standards/audit within identified clinical area
  • Evidence of continuing professional development (CPD) relevant to the clinical area
  • Evidence of specialist practice skills and able to demonstrate the impact of this on practice change/development
  • Evidence of promoting/supporting active user involvement and participation
