Job summary

The ACP is a senior autonomous practitioner within SCFT, providing expert mental health diagnosis, assessment, formulation, treatment and risk management to adults with severe and enduring mental illness who require ongoing specialist forensic follow up in the community.

Working across the four pillars of advanced clinical practice - clinical practice, leadership and management, education and research/evidence and quality improvement the ACP will act as a visible clinical leader within SCFT. Working in partnership with SCFT leadership team driving service development, workforce capability and high quality, evidence based care in line with national advanced practive, mental health frameworks and the SCFT leadership team.

Main duties of the job

The post holder will hold a complex community forensic caseload and use advanced reasoning, non medical prescribing (where appropriately qualified) and high level decision making to optimise outcomes, reduce relapse and promote recovery, working collaboratively with service users, families and carers.

The role includes providing specialist forensic advice, in-reach and consultancy to district services, supporting prevention of unnecessary admissions to secure care and contributing to safe and effective patient flow across the system.

The post holder will provide specialist forensic advice to internal and external partners, support safe and effective patient flow across the service and community pathways and participate in relevant on call or extended hours arrangements as required.

Person Specification

Education and Qualifications

Essential

  • oMaster's degree in advanced clinical practice or NHSE E - portfolio completed to support master's level advanced practice competency.
  • oRelevant professional qualification to post graduate degree level or equivalent experience in area of expertise.
  • oEvidence of continuing professional development
  • oExperience within an inpatient/community/Social Care setting
  • oExperience of successful leadership and management in a complex organisation.

Desirable

  • oEvidence of leading on collaborative working across system partners to influence system wide change.
  • oV300 non-medical prescribing

Experience

Essential

  • oSignificant experience in secure mental health services at senior clinical leadership.
  • oExperience of working in a multi-disciplinary setting
  • oExperience of successful leadership and management in a complex organisation
  • oAbility to demonstrate leadership and delivery of safe, effective, caring, responsive and well led services.
  • oKnowledge and application of key policies and procedures
  • oPresentation skills.
