Job summary

If you are enthusiastic and passionate about improving the lives of autistic people and people with a learning disability, then role as a Senior Case and LDA Commissioning Manager in our trust could be right for you. We have an opportunity for an innovative, skilled health professional or social worker to join our team who provide a commissioning service to adults and children with a learning disability and/or autism at a time when they need support from mental health services. This can be around preventing hospital admission and discharge planning, including commissioning community-based services to meet individual needs.

Main duties of the job

  • Provide case management support to autistic people and people with a learning disability who are at risk of hospital admission, or supporting discharge for these people who are currently receiving inpatient treatment., including identifying or commissioning services to meet individual need.
  • Inputting into the referral and review processes for the Dynamic Support Register.
  • Undertaking Care (Education) and Treatment Reviews

Job responsibilities

  • To provide case management to people under the LDA Programme.
  • Manage own delegated portfolio across the LDA Commissioning team.
  • To provide management and leadership to support staff working in the LDA Commissioning Team to deliver on the he LDA Programme.
  • To work with people with a learning disability and/ or autism to maximise their independence with the appropriate support and services.
  • To plan, promote and commission services.
  • To ensure the service delivery both directly and commissioned are in line with national policy and guidance in relation to the Dynamic Support Register (DSR), care, education and treatment reviews (C(E)TRs) and Case Management in a person centred and individualised approach.

Person Specification

QUALIFICATIONS

Essential

  • Professional Health or Social Care Qualification and current registration
  • Post graduate certificate or equivalent experience in leadership/ management/ or area of practice
  • Evidence of a commitment to lifelong learning

KNOWLEDGE/EXPERIENCE

Essential

  • Significant experience of working with people with a learning disability and/ or autism needs in a paid or voluntary capacity in health/social care/teaching setting
  • Detailed knowledge of legal issues pertaining to the specialist service including; Mental Health Act 1983, Children's Act 1989, Mental Capacity Act, Transforming Care, safeguarding and child protection legislation
  • Detailed operational knowledge of specialised services, including risk assessment/management
  • Knowledge and experience of resources/ systems and processes available to people with learning disabilities and/ or autism to support admission avoidance, discharge planning and community support
  • To identify issues relating to Safeguarding and risk, which may be at times be distressing and actively participate in due processes and Mutli-Disciplinary Meetings as required
  • Specialist knowledge of:- Dynamic Support Register (DSR), Care, education and treatment reviews (C(E)TR) and Case Management and the relationship across the whole system and national expectations of processes and guidance
  • Experience of managing, leading and supervising others
  • Experience of working as part of a team
