Work collaboratively within a multi-disciplinary team (MDT), contributing to shared decision-making, case discussions, and service delivery.

Support individuals in their recovery journey, promoting self-management, resilience, and access to appropriate services and community resources.

Facilitate appropriate referrals and transitions between primary and secondary care services, ensuring timely and safe access to care pathways.

Contribute to clinical governance, audit, service evaluation, and quality improvement activities, supporting the ongoing development of the LPMHSS.
