Are you looking for the opportunity to work within an award winning specialist Community Falls & Rehabilitation Team? We are looking for a skilled and enthusiastic Occupational Therapist to join our multidisciplinary team providing assessment and rehabilitation for adults with a range of conditions impacting their functional ability. Our friendly and supportive team is comprised of physiotherapists, occupational therapists, specialist nurses & therapy assistant practitioners, who are all passionate about patient centred care and supporting individuals to regain, retain & improvement their functional independence. Having won a NICE Shared Learning Award & finishing runner up in the CAHPO awards 2019 for the developments to our Falls service, the team are listed as an example of good practice: Dudley Falls Prevention Service NICE. This demonstrates our commitment to evidence-based, multifactorial assessment & interventions for our diverse and often complex caseload. As a service we have built upon this success, embedding best practice & seeing the rate of falls related admissions remain below the national & local average. The service works closely alongside the local Rapid Response Team-Own Bed Instead, and Intermediate Care Team colleagues, with opportunities for collaborative working to optimise patients rehabilitation journeys. The role will work delivering tailored, specialist treatment plans both in patients homes and from our specialist Rehab Centre at Corbett Hospital

Main duties of the job

This post is part of the Community Rehabilitation and Falls Team which provides effective and efficient multidisciplinary rehabilitation for a caseload of patients with a variety of physical conditions. Duties will include assessment, Occupational Therapy (OT) interventions such as assessment for equipment, ADL, planning therapy programmes and discharge of patients, as well as appropriate delegation of work to juniors, assistants and students. Working as a lone practitioner withinpeople's own home, using evidence based/client centered principals to assess, plan, treat and evaluate interventions.

Person Specification

Qualifications & Education

Essential

  • Diploma / degree in Occupational Therapy
  • Registered with HCPC
  • Evidence of relevant post-graduate experience

Desirable

  • Member of RCOT

Knowledge and Training

Essential

  • Awareness of relevant national and local guidelines
  • Good knowledge of falls prevention and falls risk factors as per national guidelines
  • IT Training / knowledge
  • Commitment to personal and team development

  • Willingness to work towards competencies in vestibular assessment & treatment

Experience

Essential

  • Experience of working with physically impaired patients
  • Completed rotations in core areas or equivalent rehabilitation experience across a wide variety of clinical settings
  • Experience of rehabilitation work within an acute/community rehabilitation setting
  • Relevant experience at post graduate level
  • Experience of working in a Multidisciplinary team
