An exciting opportunity has become available to expand the team ofSite Practitioner cover, Critical Care Outreach service to all level 1 wards and clinical leadership in the Elizabeth Intensive Care and High Dependency Unit at The Royal Brompton Hospital.
This service provides Site cover and Critical Care Outreach assessment and clinical leadership within the clinical group. As a site practitioner or a member of the Critical Care Outreach service you are the first point of contact for the wards, and proactively identify and treat deteriorating patients outside the ICU, improving safety and preventing ICU admissions troubleshooting deteriorating patients.
When not working within the Site Practitioner or Outreach service the Practitioner is based within the Elizabeth Intensive Care and High Dependency Unit where they will provide Clinical Leadership byacting as a Clinical Nurse in Charge, overseeing ICU/HDU patient flow, and ensuring high-quality, evidence-based care. They will provide Expert Care by managing complex cardiothoracic patients, whilst supporting the Matron with staffing, resource management, and mentoring staff/ students.
This post is an ideal opportunity for experienced critical care nurse to develop their leadership skills and expertise in Critical Care, Outreach and the hospital at night patient service. The successful candidate will lead the Hospital at Night service and weekend teams across the clinical group.

Main tasks & responsibilities:
Patient/customer care (both direct & indirect)
- Ensure that nursing documentation is maintained to the Trust standard. Under supervision participate in conducting nursing audit to achieve the agreed standards. contribute to and participate in the process of setting goals and benchmarking standards of care. contribute to developing and maintaining a ward /department ethos and provide a standard approach to the delivery of excellent nursing care. Demonstrate through a holistic approach to nursing care and day to day clinical issues an understanding of evidence-based practice. To demonstrate awareness and compliance with Trust policies, procedures, guidelines and standards. To be aware of the Trusts framework of clinical governance within the ward or department area, seeking advice or guidance where necessary. Identify and report clinical risks to the ward/departmental manager/modern matron. provide a suitable and safe environment for the physical, psychological and spiritual well being of staff, patients/clients and their relatives/carers, seeking guidance or advice where necessary. Record and report all incidents/complaints involving staff, patients and visitors in accordance with Trust policies and assist in any investigations as required. To take charge, whilst being supervised, of the clinical needs of a group of defined patients and/or ward or department on occasion prioritising workload according to changing clinical need for a shift. Collaborating with other members of the ward team to ensure the patients needs are met to a consistently high standard. Delegate tasks and activities to a range of team members in relation to patient care. (for example when taking supervised charge or as a link nurse). To actively promote a healthy lifestyle for patients and staff in line with national strategy requirements.
Communication
Demonstrate politeness, courtesy and sensitivity in dealing with patients/clients, visitors/relatives and colleagues, maintaining good customer relations. Contribute towards sharing good practice within the ward/department and Trust wide. To promote a positive image of the Trust at all times
People Management
To woActively manage own annual leave in line with Trust and local Policy and Procedure. Participate in the Trusts appraisal and person development plan schemes by meeting with the manager at least annually.Participate in the programme for clinical supervision as required.
Information management
Participate in the collection of audit and research data as required. Use data which will contribute to the improvement of patient care. Undertake relevant training for electronic information systems. Following Trust/local guidelines to access results from the Patient electronic system.
Resource management
To work together with the ward team to ensure effective use of resources To actively engage with self rostering in clinical ward or departments
Service development
To contribute to the development and maintenance of a positive learning environment for colleagues, patients and visitors. To participate in staff education and development, including utilising appropriate learning opportunities and to act as a mentor and resource for student nurses, adaptation and unqualified members of the nursing team.
Policy development
Under the supervision of the ward / department manager to work on delegated projects relating to local policy development. To be actively involved in the implementation of Trust policies and procedures at ward / department level, reinforcing their use.
