We are excited to be recruiting to our Community Specialist Palliative Care Nursing Team for North Worcestershire.
The service covers a wide area from Tenbury Wells in the west to the outskirts of Birmingham in the east. The service has two location, Princess of Wales Hospital in Bromsgrove and KEMP Hospice in Kidderminster. The advertised post is to be based at Bromsgrove, with an expectation of supporting the west side as clinical need requires.
We provide a service 365 days a year from 9am to 5pm, with weekend and bank holidays covered by a rota which the successful applicant will join once they have settled into the team.
You should be an effective team member willing to share experience and knowledge of palliative care to help support your colleagues as well as a willingness to learn and develop as the team strives to deliver excellent care.
It is essential you can fulfil the travel requirements of the post.

Main duties of the job
The successful candidate will be working collaboratively with Palliative Care services across North Worcestershire including Palliative Medicine Consultants, Palliative Care in-patient Unit, Neighbourhood Teams, specialist palliative care services and specialist services within the neighbouring acute Trusts, Hospices and primary care colleagues to provide expert advice for patients with a life limiting diagnosis. The role will include caseload management, being a primary resource to health professionals in care settings giving advice for symptom control and advance care planning. You will attend and present patients to the weekly MDT meeting and actively support the GSF meetings held by primary care colleagues. The role will also require you to undertake audit, critically appraising current research to apply to practice, updating policies to reflect these developments as well as supporting and leading research projects.

In your role you will be responsible for holding a patient caseload based around a group of GP surgeries to enable patients to live as well as possible despite their life limiting illness. You will be supporting patients and their families to make decisions about their future which allows them to be cared for and die in their preferred choice of location.

Qualifications
Essential
- Registered Nurse.
- Degree or equivalent.
- Relevant short courses e.g. Symptom Control and End of Life Care.
- Able to demonstrate the ability to teach with relevant recognised teaching qualification.
- Evidence of continual professional development.
- Willingness to undertake a Non-Medical Prescribing course.
- I understand that I will be asked to provide original certificates to verify my qualifications at point of offer.
Desirable
- Non-Medical Prescribing Qualification or willingness to complete.
- Post graduate qualification in palliative care or completion of the Princess Alice Course.
- Advanced Communication Skills (formal course).
- Recent attendance at a Pain and Symptom Control Course.
Experience
Essential
- Significant post basic experience.
- Experience of working within the field of palliative care / caring for palliative patients.
- Considerable experience in a nursing role whilst working with the multidisciplinary team; communicating effectively at differing levels.
- Considerable experience in a nursing role managing complex patients.
- Extensive experience of Caseload and / or team management.
- Experience of clinical audit and / or utilisation of research skills.
- Experience in developing and / or reviewing standards / policies.
- Experience of incorporating policy / evidence into practice.
