Job summary

We are seeking a forward thinking, highly motivated and experienced Cardiology Nurse to join our dedicated team as a Cardiology Clinical Nurse Specialist (CNS). This is a rare and excellent opportunity for an enthusiastic and knowledgeable clinician to take a senior role within the Cardiology CNS team.

The successful candidate will demonstrate exemplary clinical assessment skill, strong leadership and a commitment to high quality patient centred care. They will ensure the delivery of high quality, evidence based care for our patients, working along side an experienced CNS team and Cardiologists.

Main duties of the job

  • Autonomously manage a caseload of patients referred from GP, A&E and post hospital follow up.
  • Work closely with Cardiologists, the CNS team and Cardiac Physiologists.
  • Provide clear and concise letters to GP's, that detail the diagnosis, management plan and ongoing care decisions, ensuring good patient communication between primary and secondary care.
  • Attend weekly multidisciplinary team meetings.
  • Be competent in clinical assessment, history taking and able to formulate a clear evidence based management plan for patients with angina, ACS, heart failure and arrhythmias.
  • Request and interpret diagnostic investigations such as ECGs, blood tests and imaging.
  • Provide guidance to patients on their diagnosis, treatment plan, including medicine and lifestyle advice to ensure they understand and engage in their treatment.

Job responsibilities

  • Work independently contributing to the delivery and effective running of clinics.
  • Work autonomously within their scope of practice, escalating appropriately if needed.
  • Provide advice, mentorship and clinical supervision to nursing and MDT colleagues.
  • Contribute to staff education, training and the development of advanced practice, Working closely with the Cardiac Rehabilitation and Heart Function Cardiac Rehabilitation teams.
  • Participate in audit, quality improvements and service developments relating to ACS and chest pain pathways.

Qualifications

Essential

  • Health assessment module
  • Cardiology specific qualification or equivalent experience

Desirable

  • Motivational interviewing/Behaviour change/Counselling course

  • Demonstrate an ability to problem solve highly complex activities
  • Planning and organizational skills for a range of complex activities

  • Post registration experience in cardiology
  • Demonstrate the ability to deliver advanced clinical skills in practice
