Job summary
An exciting opportunity for Community Nurses has arisen in the Sudbury Integrated Neighbourhood Team. The team are based in a modern Health Centre in Sudbury which is a historic market town surrounded by picturesque villages. The Sudbury Health Centre is the base for a wide range of other health professionals and agencies including Adult Social Care and a GP practice so we are able to work together in a fully integrated way.
The successful applicant will have a thorough understanding of community nursing and will be joining a dynamic, friendly team of community nurses, community matrons, therapists and social care staff. You will be providing nursing services to patients in the community and have the chance to work in an autonomous manner with the support of the wider multi disciplinary team within the hospital and across the West Suffolk Alliance. You will be assessing the patients by carrying out home visits and planning and implementing a patient's individualised care plan. As part of the team you will also be delivering care to virtual ward patients.
Main duties of the job
On site at the health centre you have access to a range of treatment facilities and may be involved in delivering nursing care in specialist clinics. You will be using a wide range of nursing skills and will need to be confident in your assessment skills and in your ability to provide safe, effective treatments whilst working in a community setting. Experience of palliative care, wound management and catheter management will be an advantage.
This will be the perfect opportunity if you would like a role where you can use a broad range of nursing skills, work independently but with the support of a close knit team and work with patients in their own home environment to improve their health. The role suits newly qualified as well as experienced nurses. You will be able to develop your role in accordance with areas of clinical interest and to progress in your career, with training and education available to support this.
The post is part time with the possibility of full time as an option. You will need to be a car owner and driver. We operate a 7 day service within core hours 8am to 6pm.
