Community Mental Health Nurse - Band 5 to Band 6 Development post

We have an exciting opportunity to offer the right person the opportunity to be a part of our committed and enthusiastic team working with people who have complex needs relating to their diagnosis of dementia and mental health illness.

This post will sit within our multidisciplinary community dementia team and will work as a lead practitioner to deliver interventions to those people who have more complex or intensive needs. We aim to help people to remain safe and well at home and able to maintain the relationships that are important to them.

The post sits within the DOPMHS team and is part of the wider service model. The role involves working across both DOPMHS and the Intensive Support Service, depending on service need. This can include supporting extended working patterns when required, to help maintain continuity of care for older adults in crisis or those needing ongoing support.

The wider DOPMHS service runs from 09:00 to 17:00, Monday to Friday. The Intensive Support Service operates from 09:00 to 17:00, Monday to Sunday, including bank holidays.

As the post involves travel across the service area, the ability to drive and access to a vehicle is essential.

Main duties of the job

The role of the Community Nurse is to work in partnership with service users within the designated care group who have complex health needs. The focus is on developing care and recovery plans that support people to lead fulfilling lives.

The post holder will use their knowledge of clinical interventions relevant to the care group, including providing education, developing care plans and supporting discharge planning. They will work closely with carers, families and partner agencies to support this.

The post holder will always promote the safety and wellbeing of service users and those who support them. They will assess, plan and deliver care, provide specialist nursing advice, and carry out appropriate nursing procedures.

They will also provide clinical supervision and support to staff and students.

Job responsibilities

Clinical

  • To contribute to the management of a team caseload of clients with complex needs, using CPA principles, evidence based/client centred principles to assess, plan, implement and evaluate interventions, which will be delivered in the Dementia and Older Adults Mental Health Service.

  • To support/manage a team caseload and attend relevant MDT meetings

  • To regularly support and cover the duty rota

  • To provide specialist assessment and intervention as part of a multi-disciplinary approach, with specific responsibilities regarding the care pathway

  • To provide risk screening and assessment as well as risk management planning

  • To contribute to and facilitate evidence-based assessments and individual and group interventions

  • To contribute to the maintenance and development of the Service

  • To regularly share and supervise students on practice placement, and junior staff as appropriate

Person Specification

Qualifications

Essential

  • Registered Nurse with current registration [mental health/LD/Adult], appropriate to the job role
  • Completion of Mentorship Course/ENB equivalent
  • Evidence of continuous professional development

Knowledge/Experience

Essential

  • Significant post qualifying experience in the relevant care group
  • Experience of working alongside people with complex health problems in the designated care group
  • Experience of supervising staff
  • Experience of managing a caseload
  • Experience of working consultatively with professionals
  • Experience of conducting clinical assessments including risk assessments
  • Experience of partnership working within and across various statutory and non statutory teams and agencies
  • Experience of working within the CPA process
  • Understanding of clinical governance
  • Previous experience of working in the community
