We are the Kington & Weobley District Nursing Team, a small, dedicated team providing excellent planned and unplanned nursing care to patients in their own homes.

We aim to deliver high quality care to improve health & wellbeing and to support our patients to remain living at home. We pride ourselves in being hard-working, supportive, friendly and flexible. We are part of the North & West Primary Care Network and work closely with partners across the network.

We are based in the market town of Kington nestled within the beautiful County of Herefordshire - covering both Kington and Weobley surgeries in the north-west area of the county. This rural area has many areas of outstanding beauty and is predominantly agricultural with rolling country side for miles.

We're seeking to recruit a band 5 community nurse who shares our passion for caring for people in their own homes, strives towards excellence in community nursing and who has the ability to work well within a close knit team of supportive colleagues.

Main duties of the job

Our aim is to provide treatment and support to patients in their own homes. Whilst undertaking a holistic nursing assessment, we aim to encourage patients to remain as independent as possible, in order to prevent hospital admissions.

Due to the nature of this role, the ability to meet the travel requirements of the post are essential.

Main duties of this role include, but are not limited to;

  • Wound care including NPWT- Palliative care- Managing end of life symptoms and providing support for relatives/carers- Managing long term conditions in a community environment- CVAD and drain management- Chemotherapy disconnect- Holistic assessments- Post operative care following discharge- Wound assessments and management- Venous blood samples
