Job summary

An exciting opportunity has arisen for a compassionate and innovative individual to join the Haemoglobinopathy Service as an Advanced Care Practitioner (ACP). The ACP will play a pivotal role in the management and delivery of high-quality patient care, acting autonomously within their professional scope of practice.

As part of measures implemented by NHS England, the Sickle Cell Emergency Department (ED) Bypass we are in the process of setting a service for people with sickle cell disease to attend when they have a sickle cell crisis. The aim of this initiative is to help to address the longstanding health inequalities experienced by patients with sickle cell disease by improving access to specialist care. Healthcare professionals working in the Sickle Cell are trained in caring for patients experiencing sickle cell crises. Patients presenting with a crisis will be reviewed promptly by a clinician who understands their condition and can provide effective pain relief in a timely manner.

The successful candidate will be responsible for providing comprehensive care, including initial history taking, clinical assessment, diagnosis, treatment, and evaluation. They will demonstrate safe and effective clinical decision-making and deliver expert care as a core member of a dedicated multidisciplinary team.

The role incorporates the key dimensions of advanced clinical practice, education, leadership & management, and research. This is a full-time position (37.5 hours per week)

Main duties of the job

Always maintain professional conduct and consistently promotepositive image of North Middlesex University Hospital NHS Trust (NMUH), in alignment with the organisation's values.

Ensure that the needs of Red Cell patients are at the forefront of care delivery, meeting both trust and national standards for clinical quality, safety, and patient experience.

Demonstrate advanced clinical expertise in patient assessment,including physical examination, interpretation of diagnostic results and formulation of diagnoses, ensuring management aligns with current national guidelines.

Provide professional, clinical and managerial leadership to nursing and associated staff, supporting the delivery of a high-quality, responsive Red Cell services.

Contribute to the emergency care pathway by offering expert input and clinical support in the management of haemoglobinopathy/Red Cell patients, in accordance with national best practice guidelines.

About us

At North Middlesex University Hospital NHS Trust, we provide care for approximately 695 patients with hemoglobin disorders or rare inherited anemias, the majority of whom have sickle cell disorder (SCD). According to the 2023/2024 NHR annual report, we have the 7th highest number of patients with sickle cell disorder and the 11th highest number of patients with thalassemia (combined adult and paediatrics) in England.

We serve a diverse and multicultural population of around 600,000 people in North London, an area with a high deprivation index. Many of our patients come from single-parent families, refugee communities, and travelling backgrounds, and around 30% of the local population does not speak English as their first language.

Our Specialist Haemoglobinopathy Team (SHT) offers comprehensive adult and paediatrics services across Enfield, Haringey, Barnet, and parts of Hertfordshire. We are also proud to be a partner with The Red Cell Network (TRCN), which includes two Haemoglobinopathy Coordinating Centre's (HCCs). The Sickle Cell Disorder HCC is hosted by University College London Hospitals NHS Foundation Trust (UCLH) in collaboration with Whittington Health Trust (WH) and us. The Thalassemia and Rare Inherited Anemias (RIA) HCC is led by UCLH in partnership with Imperial College Healthcare NHS Trust, St George's University Hospitals NHS Foundation Trust, London Northwest University Healthcare NHS Trust, and Whittington Health NHS Trust.

Job responsibilities

Independently manage a patient caseload, utilising advanced clinical decision-making and extended practice skills.

Lead haemoglobinopathy/Red Cells practitioner-led clinics, taking responsibility for individualised care planning and contributing to annual reviews.

Conduct ward rounds independently and in collaboration with medical staff, as required.

Develop and maintain individualised care plans for patients within practitioner-led clinics, ensuring they are regularly reviewed and updated.

Coordinate and deliver specialist interventional procedures for Sickle Cell patients, such as patient-controlled analgesia (PCA) and apheresis, when required.

Respond promptly to the needs of inpatients by reviewing care plans and prescribing care and treatment regimens within your professional scope of practice.

Person Specification

Education & professional Qualifications

Essential

  • Registered Nurse/Practitioner
  • Post-registration qualification in a relevant specialty
  • Teaching qualification or demonstrable teaching ability
  • Educated to degree level
  • MSc in Advanced Clinical Practice or equivalent or willingness to do MSC ACP
  • Independent/non-medical prescribing qualification

Experience

Essential

  • Experience in Red Cell or Generally Hematology specialty at Band 7 level or above.
  • Experience in acute inpatient care settings
  • Demonstrated experience in leading and implementing changes in the workplace.
  • Strong leadership skills with a track record of effective team guidance
  • Evidence of applying research to inform and enhance clinical practice.
  • Experienced in teaching both colleagues and patients using appropriate, accessible language.

Skills and aptitudes

Essential

  • In-depth knowledge of current clinical developments within the Haemoglobinopathy/Red Cell specialty
  • Proficient in delivering both manual and automated red cell exchange or show a willingness to learn.
  • Competent in phlebotomy and cannulation
  • Experienced in intravenous drug administration and PCA management
