Job summary

An exciting opportunity has arisen within the Surgical Division for a highly motivated and enthusiastic Trainee Advanced Clinical Practitioner (ACP) to join our established Surgical ACP team.

This is a developmental post designed to support an experienced registered nurse to progress through a structured ACP training pathway, working towards a Master's level qualification and advanced clinical practice competencies.

The successful candidate will be supported to develop the knowledge, skills, and behaviours required to practice autonomously across the four pillars of advanced practice: clinical practice, leadership and management, education, and research.

The Surgical ACP team forms an integral part of the multidisciplinary workforce, contributing to the delivery of high-quality, patient-centred care across a range of surgical services including ward-based care, Level 1, surgical ambulatory care, and on-call activity

Main duties of the job

The post holder will work as a trainee Advanced Clinical Practitioner (ACP). The post holder will undertake a 3-year master's programme in Advanced Clinical Practice. Alongside the master's programme the ACP will work clinically alongside the doctor team, ACP colleagues and supervisors to gain clinical knowledge and experience within an ACP role to enable them to assess, diagnose and treat patients. The aim to eventually work autonomously taking responsibility and making clinical decisions in accordance with agreed clinical pathways of care and proficiency of own skill and speciality set.

You will work towards completing speciality specific competencies for the role and develop and actively maintain a personal portfolio of learning to ensure compliancy with speciality specific competencies and evidence of working at an advanced level.

CLINICAL PRACTICE

With support from supervisors, the trainee will learn to assess, diagnosis and treat of patients presenting within their care.

Evaluate response to treatments and initiate changes as required under supervision.

Work within own competence and confidence to ensure safe practice for patients and where unsure seeking out supervisors, senior colleagues or medical team for support.

Communicate effectively with patients, supporting people in making decisions, planning care, or seeking to make positive changes.

Communicate highly complex information to the Multi-Disciplinary Team (MDT).

Work collaboratively with other professionals and refer to appropriate clinicians to ensure the needs of patients and services are met.

Request and perform simple diagnostic procedures and support staff with patients who they are unable to obtain these procedures.

Request pathology tests under the guidance of supervisor or physician

Request chest x-rays on completion of level 7 clinical diagnostics and examination and completion of IRMER under the guidance of supervisor or physician.

Review of the deteriorating patient and initiation of management plans and escalation to medical team or senior colleagues where appropriate.

Establish and maintain effective communication with patients, their families and other relevant agencies and professionals across health, education and social services.

Promote clinical excellence and evidence-based approach to care by complying with the agreed policies and procedures.

Provide support and advice to other specialities to ensure the correct patient pathway.

Take an active role in Health promotion of individuals through appropriate support and education.

Analyse and act on information to proactively manage and maintain NHS standards and targets.

Maintain accurate patient records on trust documentation ensuring professional confidentiality is always maintained.

Provide clinical knowledge to clinical colleagues, patients, carers/relatives and primary care.

LEADERSHIP AND MANAGEMENT

Act as a role model by demonstrating effective leadership and expertise by maintaining credibility across the trust, the wider health care community and external agencies, ensuring a positive image of the trust is maintained.

Pro-actively initiate and develop effective relationships, fostering clarity of roles within teams to encourage productive working.

Act as a clinical role model/advocate for developing and delivering care that is responsive to changing requirements.

Continually develop practice in response to changing population.

Contribute to effective leadership.

Participate in sharing experience and knowledge to pre and post registered staff to contribute to the development of future advanced practitioners.

EDUCATION

In addition to successfully completing the MSC programme, identify and work towards completion of speciality specific competencies.

Actively maintain a personal portfolio of learning to ensure compliancy with speciality specific competencies and evidence of working at an advanced level.

Identify and take responsibility for individual learning and development to ensure ongoing professional development and advancement of the ACP role. Ensuring it reflects the breadth of ongoing development across the four pillars of advanced clinical practice.

Disseminate learning and information gathered to share good practice and inform relevant staff that provide the service about current and future developments.

Act as a role model, educator, supervisor, coach and mentor, seeking to instil and develop the confidence of others.

Ensure own compliance with regards to Trust mandatory training.

RESEARCH

Engage in research activity, adhering to good practice research guidance, so that evidencebased strategies are developed and applied to enhance quality, safety, productivity and value for money.

Evaluate and audit own and others clinical practice.

Develop and implement robust governance systems and systematic documentation processes.

Participate in professional and policy-making activities that contribute to the development of the practitioner role.

Liaise with other ACPs across the trust to achieve a corporate viewpoint on key professional issues.
