Post is for a qualified learning disabilities nurse to work within the mental health inreach team.
Work within a busy /challenging environment with offenders with learning and mental health needs.
Work alongside wider MDT to ensure safe care and management of prisoners at HMP Manchester
Main duties of the job
Support individuals to understand and engage with healthcare interventions, prison regimes, and legal processes.

Monitor physical and mental health needs and coordinate appropriate referrals.

Ensure prisoners receive appropriate reasonable adjustments in line with the Equality Act.

Advocate for vulnerable prisoners to reduce health inequalities and improve access to services.

Assist individuals in understanding information through accessible communication methods.
Work collaboratively with mental health teams, primary care, prison staff, psychologists, probation services, and external agencies.
- Participate in multidisciplinary meetings and case reviews.
- Contribute to sentence planning, risk management, and discharge/release planning where appropriate.
