Job summary

This is a Fixed Term/Secondment vacancy until 30th May 2027 to cover maternity leave.

As a Band 6 SPQ Community Nurse you will provide high -quality nursing care to patients with complex health needs in the community. You will assess, plan, implement, and evaluate care, helping people remain independent and avoid unnecessary hospital admissions.

This vacancy is a part time 30 hours per week post. Hours will cover a 7 day service which covers 08.00 -18.00.

Main duties of the job

The role involves management a caseload , supporting junior staff, team leader and working closely with the wider multidisciplinary team and 3rd sector . you will work closely with specialist nurse , social care and secondary care .

You will undertake comprehensive patient assessments, develop and review individual care plans, deliver and coordinate nursing interventions in the patients home.

Other duties include managing patients with complex needs chronic, palliative and end-of-life care, perform and oversee wound care, catheter care, medication and prioritising visits according to clinical needs.

You will coordinate care across multiple agencies and services, acting at all times as a role model and clinical resource for band 5 Nurses, Assistant Practitioners and Health Care Support Workers.

Provide supervision, mentoring and teaching, support staff development competencies assessments andcontribute to service improvements initiatives.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Qualifications and Knowledge

Essential

  • Registered Nurse (Part 1)
  • Current NMC Registration
  • Bachelor degree (or equivalent experience)
  • Pertinent postgraduate diploma (or equivalent development/experience and a willingness to undertake appropriate post graduate diploma level study. (N.B. Specialist Practice Award in District Nursing required for District Nursing post)
  • Bronze IQT

Desirable

  • Teaching qualification( or equivalent experience)
  • Recognised management qualification
  • Community Nurse Prescriber

Experience

Essential

  • Appropriate experience within community/specialty and able to demonstrate sound knowledge.
  • Able to demonstrate sound knowledge of NMC Code (2015)
  • Experience of working in a multi-professional environment
  • Experience of running a ward/area/department
  • Experience of working with preregistration nursing / midwifery students
  • Experience of formal /informal teaching and of mentoring preregistration nursing or midwifery students.
  • Understanding of wider nursing and health care related agenda.

  • Supervisory managment skills
  • Able to relate theory into practice through reflective skills
