Job summary
This role helps older people live well at home by providing expert medical care, making a real difference to their independence and quality of life. You'll join a skilled, supportive multidisciplinary team including geriatricians, nurses, therapists and social care colleagues. Together, you'll deliver person-centred care across community settings. Your work will help prevent unnecessary hospital admissions and support people with complex frailty needs. You'll also contribute to improving services and shaping how care is delivered in the community.
Main duties of the job
What you'll do:
- Provide medical assessment, diagnosis and treatment for older people with frailty
- Support urgent community response and virtual ward care
- Lead on complex case management and develop clear care plans
- Work closely with multidisciplinary teams, care homes and primary care
- Contribute to service improvement, teaching and clinical governance
What we're looking for:
- A qualified doctor with full General Medical Council (GMC) registration
- Experience in geriatric medicine or caring for older people
- Strong clinical judgement and decision-making skills
- Confidence working both independently and as part of a team
- Good communication skills with patients, families and colleagues
