Job summary
An exciting new position within the Vascular team has arisen to support patients through their Vascular pathway.
The role is intended to support patients at all stages in their care including assisting in the further development of nurse led appointments.
There is an existing team of clinical academic nurse specialists with whom the appointed individual will work closely and (with training) cross cover for other nurse led services could be developed. There is significant future potential for increasingly independent practice within the department. We would support individulas with both opportunities and resources to pursue formal post graduate qualification in advanced areas of practice relevant to the post.
Main duties of the job
To provide expert, evidence-based nursing care for patients with vascular disease throughout the acute care pathway, ensuring timely assessment, treatment, education and follow-up while working collaboratively within the multidisciplinary team (MDT). The post holder will coordinate care across emergency, inpatient, outpatient and community settings to improve patient outcomes and reduce delays in treatment.
Specialist vascular nurse role will include autonomous caseload management, clinical leadership, education and service development
