Working Full time: 37.5 hours per week

An exciting opportunity has arisen at the Countess of Chester Hospital for a permanent Advanced Clinical Practitioner (ACP) to work within theSame Day Emergency Care (SDEC) and Emergency Department (ED) footprint at the Countess of Chester Hospital.

The successful ACP will work collaboratively with the multi-disciplinary team but also work autonomously to provide, clinical care, expertise and advice supporting same day discharge's and preventing unnecessary admissions. A clinical background in Frailty would be advantageous.

Main duties of the job

The Advanced clinical Practitioner role works beyond normal professional boundaries as an Allied Health Professional (AHP) and will be working at an advanced level undertaking tasks that have been previously undertaken by medical and/or technical staff.The Practitioner will practice autonomously and also within a defined team / multi-disciplinary team to provide advice and support to patients, carers, and healthcare professionals, in accordance with local guidelines and policies.

The Trust's services are provided from three locations: o The Countess of Chester Hospital: providing 473 general and acute beds. o Ellesmere Port Hospital: providing 60 beds as a rehabilitation, intermediate and outpatient facility. o Tarporley War Memorial Hospital: a base for community services which serve the local rural population. The Countess of Chester Hospital NHS Foundation Trust employs 5,964 staff (headcount) to provide care and treatment to a population of over 400,000 people living in Chester and West Cheshire which includes Ellesmere Port and Neston as well as the Deeside area of Flintshire. Person Specification

Qualifications

Essential

  • MSc Advanced Practice/Clinical Masters with evidence of clinical competencies sign off
  • AHP or Nursing Diploma or Degree HCPC/NMC Registered
  • Continuous Professional Development portfolio and relevant clinical skills to support clinical practice.

Desirable

  • Trained in Intermediate Life Support
  • Recognised teaching/assessing qualification

Knowledge and Experience

Essential

  • Knowledge of advanced clinical, psychological, and social needs of patient care

Desirable

  • Evidence of change management and/or innovative practice
